2. Gather Your Documents Before You Arrive
Bring a valid Oklahoma government-issued photo ID such as a driver’s license or state ID card. Also bring proof of Oklahoma residency such as a utility bill, bank statement, or lease agreement. You do not need prior medical records to qualify, though you are welcome to bring them if you have them.

3. Check In and Complete Your Intake Form
