Since January 1, 2026, only doctors registered with OMMA can sign your form. This rule comes from a state law called SB 1066. Forms from doctors who are not registered get rejected. Every Okie MMJ Doctor physician is fully registered.
Submit Your Application to OMMA
Next, you apply to the state online. Start on the OMMA patient application page. Apply within 30 days of your visit, because your doctor form only lasts that long. Here is what you will upload:
- Your signed Physician Recommendation Form
- A photo ID, front and back
- Proof that you live in Oklahoma
- A clear, color photo of your face
The state fee is $100 plus a $4.30 card fee. However, some patients pay less. If you have SoonerCare, Medicare, or are a 100% disabled veteran, you pay $20 plus a $2.50 card fee. Get Your Card in the Mail
After you apply, OMMA reviews your application within 14 business days. Once you are approved, the state prints your card and mails it to you. Printing adds one or two business days, plus mail time.

What SB 1066 Means for Your Card
SB 1066 is a 2026 Oklahoma law about who can sign your form. Since January 1, 2026, your doctor must be registered with OMMA and trained. If a different doctor signed your last form, check that they are registered before you renew.
