1. Gather Your Required Documents
After attending your appointment and receiving your physician recommendation, gather everything you need before logging in to the OMMA MedPortal. Review the official OMMA Patient Application Checklist to make sure you have everything ready before you begin.
Before uploading your recommendation form, look it over carefully and confirm the following sections are complete and correct:
PATIENT INFORMATION: Full name, date of birth, address, and appropriate proof of identity.
PATIENT MEDICAL CONDITIONS: This section is 100% optional in Oklahoma.
PHYSICIAN INFORMATION: The physician will complete all of the necessary fields in this section.
If there is an error on the form, contact Okie MMJ Doctor and ask the physician to correct it. Do not make changes to the recommendation yourself.
If you qualify for the reduced state fee, also have your Medicare card, SoonerCare document, Medicaid enrollment proof, or VA disability letter ready to upload.
2. Log In to the OMMA MedPortal
Go to OMMA’s website and log in to the OMMA MedPortal with your account credentials. If this is your first application, you will need to create a new account using your name, date of birth, email address, and Oklahoma ID information. The portal walks you through each section of the application in order.
3. Upload Your Documents and Photo
Upload your physician recommendation, proof of identity, proof of residency, and your passport-style photo when prompted. Make sure all uploaded documents are clear, well-lit, and fully visible. Blurry or incomplete uploads are one of the most common reasons applications are flagged as incomplete and returned.
*Remember, the first step is always, Get Your OMMA Card Recommendation. Without a recommendation from a board-certified Oklahoma physician, you will not be able to complete your application for a medical marijuana card.
The photo is the number one reason OMMA applications are rejected. Use a plain light background with your full face clearly visible. Review the official OMMA Photo Requirements before uploading.
4. Pay Your State Application Fee
The OMMA state application fee is paid during the application process by Visa, MasterCard, or Discover card. The standard fee is $100 plus a $4.30 credit card processing fee. Patients who qualify for the reduced fee pay only $22.50 total. You must upload proof of eligibility at this step to receive the reduced rate.

5. Wait for OMMA Approval
OMMA has up to 14 business days to approve, reject, or deny your application after submission. You will receive an email notification when a decision is made.
Once approved, you can use your official OMMA approval email or your MedPortal approval status as a temporary license at any licensed Oklahoma dispensary right away — no need to wait for your physical card to arrive in the mail.

Years Your Card Is Valid
Oklahoma MMJ licenses are valid for two years from the date of approval.
Days to Renew Before Expiration
Start your renewal at least 30 days before expiration to avoid any gap in coverage.
