OMMA Renewals Begin Soon!  Wondering what you need to renew your Oklahoma OMMA MMJ card in Oklahoma?
Oklahomans age 18 and older must obtain a signed  – “Adult Patient Doctor Recommendation”.
Oklahomans under the age of 18 must get two Doctors signatures. This must be documented on the “Minor Patient Doctor Recommendation form”.
According to the OMMA, the following items are needed: OMMA’s Patient Check List.

1. New Physician Recommendation form or two signed forms for minor applicants.
2. Proof of Identity (driver’s license, passport or approved tribal ID).
3. New Photo (digital passport-style photo taken within last 6 months).

Renewal applications can not be submitted more than 30 days before the current OMMA card expires.

Once the application is submitted, it will be reviewed by the OMMA. If the application meets all requirements, a OMMA staff member will mail an approval letter with your OMMA MMJ card within 14 business days of the application date.  If the application is incomplete due to unclear, missing information or documents.  The applicant will receive a email stating the reason’s it is incomplete. The patient’s submission date is the date the application is submitted online.  If the application is reviewed and denied,  because the applicant does not meet all the requirements.
A denial letter stating the reason’s for denial will be mailed to the applicant within 14 business days.
